1. Download or complete online the new patient forms. There
are separate forms for children/adolescents and for adults. If
you print out the new patient forms to complete them rather than
completing them online, bring the forms with you to your appointment.
2. Download the insurance form and the Texas Notice. Bring
the completed insurance form and the Texas Notice Signature Page
with you to your appointment.
3. If your insurance requires precertification, please call the telephone number
on your insurance card to obtain the precertification prior to your appointment. Also
please check with your insurance company as to the amount of your copay.
