If you are considering making an appointment, thank you for your interest and confidence. I would like to make the process go as smoothly as possible. The following steps may be helpful to consider:

First, please call or email Dr. Hall (713-973-2800 / karynhallphd@sbcglobal.net) to determine if making an appointment with Dr. Hall is the best option for you and to determine if any new patient appointments are available. If it is decided that this is the best option, please follow the following steps:

1.  Download or complete online the new patient forms.  There are separate forms for children/adolescents and for adults.  If you print out the new patient forms to complete them rather than completing them online, bring the forms with you to your appointment.

2. Download the insurance form and the Texas Notice.  Bring the completed insurance form and the Texas Notice Signature Page with you to your appointment.

3. If your insurance requires precertification, please call the telephone number on your insurance card to obtain the precertification prior to your appointment.  Also please check with your insurance company as to the amount of your copay.

4. Use the link Schedule Appointments for making your appointment online.  If this is unsuccessful for you  call or email Dr. Hall and she will make the appointment for you.
